Needs Assessment and Service Co-ordination is the entry into publically funded Community Support Services for: |
 |
people over 65 years who have a disability that will last longer than 6 months and results in a reduction of independent function to the extent that ongoing support is required |
 |
people [over 18 years] who have a need for short term [4 -12 weeks] personal health support – e.g. following surgery or an admission to hospital |
 |
people needing Meals on Wheels |
 |
people who need home based services who have a palliative care diagnosis |
 |
people aged less than 65 years who have a chronic health condition long term disability and very high support needs as a result of chronic disease who meet the eligibility for the Long Term Services Chronic Health Conditions [LTS –CHC]. This service was previously known as the Interim Funding Pool [IFP]. |

The assessments may be carried out in the client's own home or when they are in Middlemore Hospital/Manukau Surgery Centre or in Residential Care, either at rest home or hospital level. The assessment is to evaluate the needs, strengths and preference of the person. The assessment tool used is interRAI - an internationally validated needs assessment tool mandated for use by NASC services by the Ministry of Health. |

NASC has a range of professional clinical Needs Assessors and a wide cultural mix. Please advise if the client prefers an Assessor of a specific ethnicity and we will do our best to oblige. |

| Charges |

Free for New Zealand citizens and residents who meet the NASC eligibility criteria for publically funded support services. Proof of residency will be required and asked for. |

Non New Zealand residents – do not qualify and are not entitled to access publically funded services but can contract privately for services by contacting a provider. |

To receive long term publically funded Household Management [HHM] assistance clients must meet the eligibility criteria and are required to hold a current Community Services Card. For clients who do not meet the eligibility criteria for publically funded HHM support these services can be contracted by the client through private providers. See link above. For Short term HHM assistance under Personal Health, HHM may be provided for up to 12 weeks if the clients meet the eligibility criteria. |

NASC do not provide publically funded gardening/lawn mowing services. These services may be contracted by clients through private providers or in some cases Work and Income may be able to assist. |
