Needs assessment and service coordination

The primary point of contact for services for older adults is Needs Assessment and Service Coordination (NASC). NASC services cover help to stay at home, support services for carers and entry into residential care. They can also link with other District Health Board (DHB) services and community based agencies.

  • District nursing: Phone 04 570 9148, Monday to Friday 8am to 4.30pm, with reduced service on evenings and weekends. 
  • Shuttle services: Wellington Free Ambulance Patient Transfer Service: available in greater Wellington and Wairarapa areas, phone 04 806 0048, 8am-4.30pm. After hours 0800 426 285. Provide non-urgent transfers for disabled people between rest homes and hospitals, transport to and from hospital for scheduled treatments like dialysis. Despite its name, there is a cost for this service. Upper Hutt RSA: Phone 04 529 0016 10am-midday, Monday to Friday. The RSA operates a vehicle that can be loaned to transport returned and service people and their immediate family to attend medical appointments.
